Subject: Request for Medical Leave and Academic Consideration
Dear Associate Dean,
I am writing to request a medical leave due to my continuing and extenuating medical circumstances, this current academic semester. I am a student in the Faculty of Computer Science, and as per the recommendation from my doctor, I need time to return home to attend to my health.
My student record should show my ongoing medical challenges since my surgery in November 2019. Treatment courses needed to be adjusted multiple times since, and now once again. As a result of my new treatment plan, I have been unable to continue with my studies this semester. Therefore after careful consideration, my doctor believes that taking a medical leave for this semester is necessary for my well-being and to promote healing.
I am requesting that I be allowed to drop all of my current classes. I have attempted to do so the past week, however, I was unable to make any changes on Solus. After reaching out to Admissions, Student Records and Services and The Arts and Science Faculty, I was directed to you.
I would be grateful if you could grant me this request, at the advice of my doctor, and provide me with the necessary time to support my healing. It is well documented that various academic accommodations have been provided to me in the past and my transcript will reflect that these accommodations have unfortunately not been sufficient. I am hopeful that with this medical leave, I will be able to return to my studies in the Fall 2023 semester, ready to continue my academic journey.
Attached, you will find my doctor??s note from January 2023, which I was unable to submit due to my circumstances. My doctor??s recommendation for a medical leave will be submitted shortly. Please advise should there be any other documentation or forms required.
