To whom this may concern,
My name is Evan Huys. I am a third year student in the Concurrent Education program. At this time, I am petitioning to extend my Incomplete Grade status for INDG 301 (Fall, 2022.)
I am petitioning to extend my Incomplete Grade status for INDG 301 (Fall, 2022) because I have extenuating medical circumstances beyond my control. I have been consulting medical professionals (including my family doctor, my psychiatrist and a phycologist) and trying out treatments to address an ongoing/unresolved mental illness. I have been unable to complete the incomplete assignments in the standard incomplete grade timeline.
The extension of my Incomplete Grade status for this course will hopefully allow me enough time to complete the overdue assignments and receive the credit I have been working towards.
