Submit an Academic Petition or Appeal

Petition Letter: I have had an ongoing personal struggle with my mental health all semester which has severely impacted my attendance all semester. I have been diagnosed with anxiety since grade 9 and it has been an ongoing struggle for me but have recently also felt depression symptoms. I have consulted with my doctor about it about 4 or 5 months ago and have been referred to other doctors. I have also had therapy appointments through the schools mental health services but have not found successful treatment path for myself yet. This is taking time as a process and I really feel it has affected this course the most out of any course I have taken. This course definitely lowered my self esteem because I don??t feel I am good at math which would further exacerbate my anxiety around the class and my attendance which has made it extremely difficult for me to be able to complete it. I want the opportunity to do well and I am in contact with my doctor about next steps for this course as it is something I have to take for my degree but I would appreciate any support you could have for me during this time. I want to be successful during my time in university and I feel if this goes on my transcript it is going to really lower my gpa which will further exacerbate my anxiety
