Petition Letter: Dear Dr. Jenn Stephenson,
I am writing to request permission to extend an Incomplete grade for STAT263 and CHEM282. I I was unable to write the deferred exam earlier this semester due to a concussion. I understand that I need to submit an appeal to request permission to write the exams at a later date, and I hope that you will consider my appeal.
As per the policy of the university, I was scheduled to write the deferred exams earlier this semester. Unfortunately, I sustained a concussion among other injuries, which prevented me from writing the exams at that time. I was advised by the university to submit an appeal to request permission to write the exams at a later date, which is the reason for this email.
The concussion caused me to miss classes for two weeks, and I have been struggling to catch up and keep up in my classes ever since. After speaking with health-care professionals, I was told to get assessed for post-concussion syndrome. I have been greatly struggling with motivation and concentration which has made it extremely challenging to stay on top of my coursework, and I have been working hard to stay on track despite these challenges. I am currently being treated by the queen's concussion specialists for this as well.
I have attached the documentation regarding my concussion to this email, and if needed, I would also be able to provide documentation proving that I am being treated for post-concussion syndrome. If necessary, I can also follow up with my family physician who I have been communicating with regarding my struggles with motivation and concentration since my concussion. I have also consulted with my academic advisor about how to write this appeal, as this has been an extremely stressful situation.
I am passionate about these courses, and I believe that I can perform well if given the opportunity to write the exams. I am willing to make up for any missed coursework and to adhere to any conditions you deem necessary for me to complete the courses successfully.
If an Incomplete grade is extended, it would provide me with the opportunity to finish these courses during the summer. I have deliberately opted out of working or taking any summer courses to devote my full attention to these two classes. Furthermore, I will continue to receive treatment from the concussion specialists at Queen's University to mitigate the symptoms of post-concussion syndrome, which will enable me to take these exams in a manner that accurately reflects my comprehension of the course material.
