Petition Letter: Hello Dr. Crow Associate Dean,
a. Request: I am making this petition to request late course drops in BCHM 316, BCHM 313, CHEM 222 in both the 2022 and 2022 winter terms as well as BCHM 317 in the 2022 winter term and ANAT 100 in 2023 winter term.
b. Explanation: I have a chronic medical condition which has been developing and complicating lately. It has impacted my academics as I am not able to attend in person classes and am limited in academic work when experiencing an episode of my condition. My condition was worse last year experiencing a higher frequency of episodes which impacted my academics all year last year at queens. Last year was my first year at queens as I transferred from Laurentian university. My first year at queens I was unaware of QSAS (queens student accessibility services) and did not document my condition with them as I wasn??t familiar with the procedure at queens. I got academic considerations for each episode my first year at queens and was able to finish the first term well and in the second term of 2022 my condition worsened and I wasn??t able to really Finnish my courses in the second term of 2022 which is why I am requesting a late request for those courses in 2022 winter term. Then in the summer I took a course BCHM370 and it was very successful. In addition I found out about QSAS and now my condition is documented by QSAS and I received my accommodations. Then the first term of 2023 school year was relatively successful but I was still experiencing episodes several times a month. Then in the second term or winter term of 2023 my health was going good for several weeks with not many episodes of my condition but around week 6 I started experiencing higher frequency of episodes and some new symptoms which greatly impacted the second half of the winter term as well as final exams. Around this time, I went to student wellness services and have had a great experience with a physician at queens at student wellness services who has helped me with some new medication options as well as some new updated documentation about my condition.
c. My plan moving forward is to take another summer course as it went well last summer first. Next to retake BCHM317, BCHM 316, BCHM 313, CHEM 222, and ANAT 100 in the following year which puts me on a trajectory to graduate in 2 more school years. With my new physician at queens from student accessibility services and a new medical approach as well as the accommodations at QSAS I am optimistic that I can stay healthy enough to accomplish this with the help of the accommodations from QSAS
