Dear Dr. Jenn Stephenson,
Subject: Academic Petition to Drop CISC 223, Software Specifications - Fall 2022
I am writing to submit an academic petition to drop the course CISC 223, Software Specifications, for the Fall 2022 semester, taught by Prof. Kai Salomaa. This petition is necessitated by extenuating mental health circumstances, for which I am currently in the process of obtaining accommodations through QSAS.
My medical condition has had a profound effect on my ability to attend classes, submit assignments on time, and fully engage with the course material. As a result, I have struggled with several late or missed assignments and performed poorly on the midterm exam due to my inability to attend classes or office hours. In December 2022, I sought Academic Consideration to defer the original exam, as my symptoms intensified. Unfortunately, without access to mental health services, my symptoms continued to worsen, and the looming exam further aggravated my mental health, making it impossible for me to adequately prepare. Consequently, I was compelled to defer the exam again, obtaining medical documentation from a family doctor to use for Academic Consideration.
I had a conversation with the Professor and requested the possibility of contacting him during the Winter 2023 semester to write the exam, to which he kindly agreed. Regrettably, I did not follow up with him as I found myself overwhelmed by my academic responsibilities and unable to dedicate the necessary time to prepare for and write the exam. In the end, it became evident that the most suitable course of action was to drop the course.
Having deferred the exam twice with the hope that my circumstances would improve and allow me to successfully complete the course, I still find myself unable to adequately prepare for and write the exam. Therefore, I kindly request your permission to drop the course and retake it with the necessary accommodations, as it is an essential component for my graduation.
In an effort to address my mental health concerns, I sought support from a therapist through Student Wellness in January 2023. This eventually led to a consultation with a family doctor in March, who then referred me to a psychiatrist I met with in April. I have follow-up appointments scheduled with both the family doctor and psychiatrist in May, and I remain optimistic that these consultations will result in obtaining accommodations from QSAS and the necessary mental health care to alleviate my symptoms. Meanwhile, I managed to secure short-term accommodations through QSAS from February to the end of April, which greatly contributed to my success in my current courses. In support of my petition, I have attached medical documentation from the counselor I consulted at Student Wellness.
In conclusion, I humbly request your understanding and consideration in granting me permission to drop the course CISC 223, Software Specifications, for the Fall 2022 semester. I am confident that this decision will allow me to concentrate on my mental health and recovery, ensuring a successful return to my studies with renewed dedication and focus in the future. Additionally, dropping this course will provide me with the opportunity to retake it with the necessary accommodations, which are crucial for my graduation.
