Submit an Academic Petition or Appeal

Petition Letter: On May 11, I was involved in a head on collision and my only vehicle was totaled. I was deemed not at fault and was seen by EMS at the scene. I visited my doctor the next day and was diagnosed with multiple injuries and concussion symptoms. Enclosed please find my doctor's note advising that I take one week off (May 13-19). Not only did I sustain injuries, but I also missed work and have had difficulty sleeping and driving since the accident. I am suffering from anxiety and have been in discussions with my therapist about strategies to manage and mitigate stress. Unfortunately, I have simply fallen too far behind in COGS 100 (which is a condensed 6 week offering) to catch up, and am unable to complete the course work on time and to my academic standards. I believe I will be able to catch up in my other two courses- SPAN 111 and IDIS 199, but not COGS 100. In addition to my car accident, I am currently involved in an extremely contentious divorce proceeding for which I have a court appearance on May 26, and for which I have been receiving ongoing counselling (court documents also attached).
I appreciate your consideration and support in this matter and plan to enroll in another course in July to replace COGS 100 in order to keep on track to graduate at the end of the summer. Should you require any further documentation please do not hesitate to contact me.
