Petition Letter

Petition Letter: I am appealing to have the incomplete grade from HIST295 removed from my transcript. I took this course in Fall 2014, and had it extended until April 2015, but was still unable to complete it.

I have been very sick since the end of August of 2014. I have been unable to complete any courses
this school year; my student file should contain fee amendment forms from this past year. The
history course that I am appealing to have removed from my transcript was a first semester course ¿¿ I had it deferred to April of 2015, but have still been unable to finish it due to my health condition, which has not gone away. I was also unable to finish my honours thesis, which was due at the beginning of April, and that I must now continue to work on throughout the summer. In addition to completing my thesis, I must also earn money and write the GRE exam this summer ¿¿ even if I was completely healthy, these would still be challenging tasks. Given the unchanged state of my health issue, my doctors, thesis supervisor and I think it is best for me to drop the history course for my health, stress levels, and so that I can succeed in the other aforementioned tasks. Please note that I have tried my best to keep up with my academics this year, and I have done everything possible to relieve this medical condition in order to do so.

Having this course removed from my transcript would also allow me to appeal for a tuition refund. Because of the many courses I¿¿ve had to drop due to my health condition, I must now take another full year, which means another full year of tuition and living expenses. In addition to this, I am now only able to work on a part-time (or less) basis, and I am physically unable to work full-time and earn the same amount of money as I usually would. Given all of this, I would ask you to please consider this appeal in order to allow me to request a full refund for this history course.

The documentation I have attached is a letter from one of my doctors, a respirologist, whom I have seen most recently out of all of my doctors. If this documentation does not suffice, I can also provide documentation from 3 other doctors, disability services, and my thesis supervisor. Thank you.

Academic Plan

Academic Plan: I am planning to complete my degree in the 2015-16 school year. Completing my degree will require taking 2-3 courses each semester. Ideally I will not be sick during this time, however, if I still am, the light course load will make it easier for me to succeed. I plan to keep in touch with disability services and my academic counsellor in case my condition persists or returns, in order to ensure that I can succeed in my studies. I also have a great deal of support from other services, various professors, and friends and family -- I will continue to upkeep these relationships in order to increase my chance of success in my studies.

In terms of my physical health, I will remain aggressive with treatments, appointments, and any other steps that will make me healthy, which will better allow me to succeed in my studies.
