Petition Letter: To the Office of the Associate Dean,

I am writing to request a late drop for Physics 115 class and to explain the circumstances surrounding my request. My performance during the final examination for Physics 115 was hindered by a migraine and relapse in depression and anxiety symptoms. I was experiencing migraine issues before the exam while trying to study and throughout the examination. I am currently on medication to reduce the frequency and severity of migraines. As well as medication for managing depression and anxiety. However, due to an issue with receiving my medication from the pharmacy, I missed a dose at an unfortunate time which I believe provoked a relapse and consequently also worsened my ongoing migraine. My family doctor is back in my hometown, making appointments and communication more challenging. My doctor sent over my prescription to the pharmacy but after I reached out to the pharmacy on the phone, I was told they had not received my prescription. I believe this was an issue with the online portal because when I contacted my doctor’s office, they assured me it had been sent. While I managed to obtain my prescription, it was a day late and I missed a dose due to the confusion. These circumstances impacted my ability to study for the final exam as well as my performance during the exam and as the final exam is worth 55% of the grade, I am petitioning to late drop the class. I can provide medical documentation from my doctor to support my chronic migraine condition, anxiety, and depression.
