Petition Letter: To whom this may concern,
My name is Evan Huys. I am a fourth-year student in the Concurrent Education program. At this time, I am petitioning to extend my Incomplete Grade status for HIST 473 (Fall, 2023.)
I am petitioning to extend my Incomplete Grade status for HIST 473 (Fall, 2023) because I have extenuating medical circumstances beyond my control. I have been consulting medical professionals (including my family doctor, my psychiatrist and a psychologist) and trying out treatments to address an ongoing/unresolved mental illness. I have been unable to complete the incomplete assignments in the standard incomplete grade timeline.
After communicating with my professor for this course, Professor Laila Haidarali, we had agreed upon a deadline for my outstanding work. Unfortunately, due to my circumstances I have been unable to meet this deadline and will require additional time. I have been trying my best to complete the overdue assignments in the timeline of the original incomplete grade, however, my poor health has made it unfeasible to do so.
The extension of my Incomplete Grade status for this course will allow me the extra time I need to complete the overdue assignments and receive the credit I have been working so hard to ascertain.
In addition, I realize I have missed the petition deadline for extending Incomplete Grades by a matter of days. I was in the process of moving home from university which took quite a toll on my health, and therefore I missed the deadline. As to why I didn’t submit this petition earlier than the end of April, my mental health was extremely poor. It was my plan to complete the form before the deadline with assistance from my family at home.
