Petition Letter
Petition Letter: I am writing to request an appeal against the Faculty's decision that I must withdraw from Queen's for a year.
I wish to be allowed to continue university in the 2015/2016 academic year.
I have had a difficult time focusing on my studies, completing my work, and have also been fighting situational depression and anxiety that have further hindered my ability to reach my full potential at school. I was speaking with counsellors at LaSalle, however, this was not sufficient. Even after having received extensions on certain assignments, I struggled with psychological factors that had a direct affect on me handing in said assignments. Because I was unable to focus on school, I fell very far behind in my studies and had a difficult time trying to get back on track. I have been struggling with the ability to remain attentive in my work since high school. Queen's became more challenging, however, I was able to get by up until January of this year. Due to an incline of familial issues, I became severely depressed and anxious the Winter 2015 semester, which made it even more troublesome for me to try and stay focused in school.
Psychologist Dr. Michael Allan will be following up with you on Monday, June 15th, and will be providing a diagnostic report that will further outline the extenuating circumstances I have been dealing with during my time at Queen's.

Academic Plan
Academic Plan: Dr. Michael Allan has helped my family and I devise a plan for my future academics:
If my appeal is granted, I plan to take a reduced course load and use the appropriate resources that will help me get back on track. This includes meeting with a personal counselor regularly, registering with the appropriate accommodations at school, determining the best medication for my situation and checking in with a medical professional. I am more confident in my academic abilities after having met with Dr. Michael Allan, after having determined a diagnosis, and after discussing potential steps for the future.
In his confidential psychological assessment, Dr. Michael Allan has proposed a ten step series of recommendations/treatment targets. Among these are a daily medication regime for ADHD.
I understand Dr. Michael Allan will be in touch with your office directly. His telephone number is 416-487-5511.
