Petition Letter: Subject: Petition for Academic Consideration Due to Extenuating Circumstances

I hope this letter finds you well. I am writing to formally request academic consideration due to significant extenuating circumstances that have impacted my performance during the period from winter in 2023 to summer in 2024. I would like to provide a detailed account of the situation, including the timeline, severity, and the steps I am taking to address these challenges moving forward.

Nature of Extenuating Circumstances:

Over the past 9 of months, I have been dealing with eye disease, I still haven't been diagnosed with what eye disease I have. During the illness, my eyes were sensitive to light, tearing, red and swollen, and itchy to a great extent, which made it impossible for me to look at electronic devices for a long time, and I couldn't concentrate on studying. In addition, my psychology also changed, and I became very anxious because I couldn't get effective diagnosis and treatment. This situation has significantly affected my ability to maintain my usual academic standards.

Timeline:

The issues began around December 13, 2023, when I noticed that my eyes became extremely red, itchy and sensitive to light. Since then, from December 2023 to February 2024, I went to CDK three times for treatment and medication, but there was no effect or improvement. Then from March to April, I changed a medical institution, Telemed MD, and received treatment from there, but the results were similar to the previous times. Then I contacted a specialized eye center, which is the medical institution where I received treatment during the summer vacation and now, Galen Eye Center. I contacted Galen Eye Center on March 18, and then I made an appointment for Diagnostic Testing on April 17, which was the fastest. Then on May 10, I met Dr. Delan Jinapriya according to the appointment. He did not diagnose what disease I had. He just gave me a medication for eyes and asked me to go home and try it. If it works, he can give me a diagnosis. If it doesn’t work, I need to continue treatment and try other drugs. On May 31, I received a call to follow up and told them that it didn’t work well, and then they provided me with a follow-up appointment. Testing on September 18 and another meeting with Dr. Delan Jinapriya on September 24. This is my entire timeline, and I also provided the proof of my treatment at these medical institutions, as well as the certificate and appointment information I got at Galen Eye Center. Despite my efforts to manage these circumstances, they have continued to affect my academic performance.

Severity and Impact on Academic Functioning:

The severity of these circumstances has been profound. This disease made it impossible for me to complete some homework and all the exams for a long time. I couldn't concentrate. Secondly, I couldn't go to class because I had to stare at the screen for a long time in class, which I couldn't do. I missed a lot of deadlines and this affected my academic plan, which is very important. Because I originally wanted to major in computer science, but I experienced too many difficulties in my sophomore year, both physically and mentally. This disease caused me to fail almost all my computer science courses, and I am now majoring in statistics. So now I can only graduate with a bachelor's degree in statistics, which is not quite what I expected. As a result, my grades have suffered, and I have found it challenging to meet the expectations of my coursework.


Request:

Given the extraordinary nature of these circumstances and their impact on my academic performance, I respectfully request the following points:

1.I need to drop the CISC 124 for fall term, since I cannot take final at that time.
2.I need to drop some of the courses in winter and I want to continuing finishing some of the courses in winter, since I was completing some of the assessments in those courses.

3.If possible, I hope I can take more credits outside the school, because I am now assigned to the statistics major, if I need to graduate successfully, I need to take a lot of credits in the next two years. So I want to apply for some credits outside the school next summer. Because of the disease, I missed a lot of credits in my sophomore year, so I need to take some credits outside the school so that I can graduate normally.

Academic Plan: Steps Taken to Address the Impact:

In response to these challenges, I have taken several steps to mitigate the impact on my academic performance:

1.Seeking Support: I have next two appointments later with Galen Eye Center, I think the next two examinations and treatments should be able to cure my symptoms or alleviate them a lot. If they still can't be cured, I think I will immediately stop all my studies and fly back to China for treatment. Because if this disease is not cured, it will cause high intraocular pressure, glaucoma, decreased vision, or even blindness, with very serious consequences. This is necessary for me because it has really greatly affected my physical and mental health. If this really happens, I will communicate with the international consultant to see what we should do academically.

2.I am currently taking some helpful medication to help relieve my symptoms so that I can focus on studying for a short period of time, and it is not that cold now. The doctor said that I will be better in the summer, but this must be treated to prevent worse consequences.

I am committed to improving my academic standing and am confident that with the appropriate adjustments, I will be able to achieve the required academic standards in future terms.

New Information: Hi, please allow me to add new information here:

After the testing and the meeting with the doctor on September 24, I was finally diagnosed with allergies and inflammation of the eyes with very severe dry eyes, and unidentified eye mites. The doctor then gave me 3 kinds of eye drops and a wet wipe and told me to follow the doctor's instructions to use them for a period of time. They said that there should be no problem this time, if there is, reach them back. I am currently receiving treatment and feel well. I have attached the supporting documents for your review.
