To whom it may concern,
My name is Danica Wall, I am appealing to change my grade for CHEM497 in Winter 2021. Due to a flare up of a mental health disorder I was unable to complete the course work as scheduled. I submitted a short-term academic consideration form for February 3rd, 2021, and later in the semester registered for QSAS.
I have finished the deliverables for this course and they have been graded. If any further documentation is needed, please do not hesitate to contact me so that I can provide any additional documentation that would be required for a successful appeal, such as a doctor’s note or note from my counsellor.
