Petition Letter: To whom it may concern:

I am writing to formally request a second exam deferral for CHEM114 and a late drop for my psychology course. This follows the advice of the Chair of Undergraduate Studies, and I deeply appreciate your time and understanding of my situation.

During the summer term, I was enrolled in CHEM114 but experienced a severe exacerbation of symptoms related to my schizoaffective disorder. This flare-up, triggered by traumatic family events, significantly impaired my ability to concentrate, regulate my anxiety/panic, and sustain motivation. Despite my best efforts to prepare, I was unable to sit for the initial exam during the summer term or the deferred exam in the following period, as my symptoms persisted.

At the time, I attempted to resolve this by contacting my accommodations advisor, but I did not receive a response. Since then, I have been reassigned to a new accommodations advisor, Toni Thornton. I also attempted to communicate with my course instructor, Dr. MacConnachie, but later learned she had transitioned to a position at another institution. I have since been informed by the chemistry department that a second deferral for CHEM114 is supported, and the exam will likely be rescheduled for the summer 2025 term.

Given the above, I am formally requesting approval for a second deferral of the CHEM114 exam. I am committed to completing the course now that my health has stabilized and would greatly appreciate the opportunity to do so.

Additionally, I had deferred my psychology exam for a second time. However, as the deferred exam was scheduled during the same period as the CHEM114 deferred exam, I was unable to write it due to my ongoing symptoms. Unfortunately, I have been informed that the psychology department does not support an additional deferral. Therefore, I am requesting a late drop for this course. My transcript currently reflects an
