Petition Letter: Dear Associate Dean,
My name is Mikaela Naumann, and I am writing this appeal to request an assistance in meeting the requirements for Chem 281, taken Fall 2024. I missed the deferred exam due to my documented medical disability. I am a Type 1 diabetic and experience a low sugar episode which could be life threatening if not treated and in the interim, makes me ill and unable to legally drive (below 5 mmol). I have been registered with QSAS and appreciated accommodated as required through the past years and have attached documentation of my disability and my low sugar reading.

Academic Plan
Academic Plan: I am excited to be completing my BSc this semester as I hope to apply to begin my Masters this coming year so I am hoping that I can arrange another deferred exam date, get an incomplete noted on my transcript and rewrite during the typical exam period this coming spring, or have my completed grades averaged for a final. Thank you I understand and accept there may be additional fees attached to this. Thank you for this consideration.
