Dear Associate Dean,

I hope this letter finds you well. I am writing to formally request an extension of my Incomplete Grade (IN) for MATH 121, allowing me to defer my final exam until the Fall 2025 term. Due to ongoing medical circumstances, I am unable to return to Queen’s University before September 2025 to complete the exam. I sincerely appreciate your time in reviewing my petition.

The Request
I am requesting an extension of my Incomplete Grade (IN) for MATH 121, originally scheduled for completion by August 2025. Due to extenuating medical circumstances, I am unable to return to campus before Fall 2025, and therefore, I kindly request an extension that would allow me to complete the exam in September 2025.

Explanation of Extenuating Circumstances
Since 2023, I have been experiencing severe anxiety, which has significantly impacted my academic performance. In 2024, I sought medical support through Queen’s University Student Wellness Services, where I underwent psychological counseling and medical consultation. However, my symptoms worsened over time, making it increasingly difficult to manage my coursework. Following a thorough evaluation, the doctors at Wellness Services strongly recommended that I return home for extended psychological treatment.

Currently, I am residing in China, where I am receiving regular psychological counseling and medical treatment to manage my condition. My healthcare providers have advised that I continue treatment before resuming my studies, as premature academic stress could negatively impact my recovery. Due to the ongoing nature of my treatment, I will not be able to return to Queen’s University before the Fall 2025 term, making it impossible for me to complete my MATH 121 exam before August 2025.

Given the unforeseen and severe impact of my medical condition on my academic progress, I respectfully request an extension of my Incomplete Grade (IN) to allow me to take the exam in September 2025, once I have returned to campus and am in a better position to succeed academically.

I have attached relevant medical documentation to verify my condition and ongoing treatment. Please let me know if additional documentation is required.

Future Plan
To ensure my successful return to academic studies, I have been actively working on managing my condition through regular therapy and medical treatment. My healthcare providers and I have established a structured plan to facilitate my recovery, which will enable me to reintegrate into my academic responsibilities in Fall 2025.

Once I return to Queen’s, I plan to:
1.Continue seeking support through Student Wellness Services and Queen’s mental health resources.
2.Maintain regular counseling and medical check-ups to ensure my condition remains stable.
3.Adjust my academic workload if necessary to accommodate my recovery process.
4.Work closely with academic advisors to establish a structured study plan for completing my coursework effectively.

I am deeply committed to resuming my studies and successfully completing MATH 121. I believe that with this extension, I will be able to fully prepare for the exam and achieve my academic goals.
