Subject: Academic Petition – Request for Grade Review Based on Misfiled Assignment Submission
Dear Associate Dean,
I am submitting this petition to request a review of my grade for the Annotated Bibliography assignment in LIBS 300, on the basis of an honest administrative error that led to an ungraded submission.
In November, I mistakenly uploaded my completed Annotated Bibliography to the Reflection Paper drop box on the course’s onQ page instead of the correct submission folder. The assignment was completed and submitted before the deadline, and I have retained timestamped documentation confirming this.
Upon discovering the error, I contacted Professor Sedge multiple times — on February 23, March 18, and May 8 — to explain the situation and provide the submitted document along with evidence. Unfortunately, the grade for the Annotated Bibliography remains recorded as zero, and I have not received confirmation that the matter will be addressed.
While I take full responsibility for the misfiled submission, I believe this was a genuine, good-faith mistake made under normal student workload pressures, further complicated by the instructor strike, during which timely communication and grading may have been disrupted. I have made every effort to communicate transparently and respectfully throughout the term.
Given that the work was submitted on time and was available for review, I am petitioning for the opportunity to have the Annotated Bibliography formally assessed and reflected in my final grade.
If this petition is successful, I will ensure such errors do not occur in the future by double-checking submission folders and maintaining clearer documentation of all coursework.
