This appeal is intended to explain the circumstances which caused me to miss the 21-day deadline for appeals.
I was diagnosed with depression in 2011, and have continued to suffer from it. Depression is an illness which can make it difficult to function as a person is expected to function. It has specifically interrupted my studies on many occasions. Furthermore, it prevented me from appealing on time. I felt hopeless, and lacked motivation to pursue this appeal. Once I stabilized on my medication and began counseling in late June, I was finally ready to appeal.
I resumed medication in May 2015, and have been receiving counseling from Tonya Bauerman at Queen¿¿s Counseling. We believe that I will be able to be successful at my studies this year, and I now wish to appeal to return to studies. I will continue with medication and counseling throughout the school year, and I will be successful in my studies this year.

I am appealing the Faculty's decision that I must withdraw from Queen's for a minimum of three years.
In 2011, I was diagnosed with Depression. This has been an ongoing, recurrent illness which has significantly affected my studies.
During March 2011, I was put on a trial of medication. In September of that year, in consult with my doctor at the time, I decided to discontinue the medication. It seemed to have had the desired effect and we decided it was no longer necessary. By December 2013, my symptoms had returned, resulting in a drastic decline in motivation and concentration. I fell behind in my studies to such an extent that I could not recover. I was unable to function. I continued to suffer in my studies through 2014, resulting in more failed classes.
Depression is an illness which makes it difficult to seek help. However, in May 2015, I was finally able to find help at Health Services. I went back on medication, and started seeing Tonya Bauerman for counseling. I will continue to receive counseling on a monthly basis, and will stay positive.
If I am allowed to continue my studies at Queen¿¿s, I would only need to complete four courses: CISC223, MATH121, and any two other half-credit courses. I intend to graduate with a 3-year General Degree Program, and would not be coming back in Fall 2016. My plan is as follows: continue my studies on a part-time schedule; stay on medication; continue to receive counseling at Health Services; and engage in productive activites such as exercise and studying between classes to keep my attitude positive.
I would greatly appreciate this opportunity to finish my degree and move into the working world.

Academic Plan
Academic Plan: My plan for this year is to continue studies on a part-time basis. I intend to take only four classes, as outlined in my appeal letter. I would have no more than 3 courses per term.
In order to prevent further problems with my illness, I will continue to receive counseling and medication at Health Services. I will also continue to engage in productive activity such as exercising and will study between classes to keep my mind active and my attitude positive.
