Petition Letter: Dear Associate Dean,
My name is Elizabeth Bartolo, student number 20364629, enrolled in GPHY under the Faculty of Arts and Science. I am writing to request that my late petition regarding the Requirement to Withdraw be accepted, despite missing the 15-business-day deadline from the letter I received on May 21, 2025.
At the time of the original petition deadline, I was out of the country and unable to meet with my healthcare physician. Since returning, I have been formally diagnosed with ongoing medical circumstances that have significantly impacted the performance of my academics for the past 3 years. Because I was away, I was not able to get the proper documentation and complete the necessary medical testing in time to submit the petition. I have attached proof of my time away, and I can provide further documentation from my physician confirming my diagnosis and treatment plan (including medication).
These circumstances not only prevented me from submitting my petition earlier but also affected my ability to meet the GPA requirements for good academic standing. I understand the seriousness of my situation and have taken the time to reflect on the necessary changes that need to be made. With medical support and an academic plan in place, I am committed to returning to my studies and performing to my full potential.
I respectfully ask that the Faculty allow my late petition to be heard. Thank you for your understanding and time.
