Petition Letter: Subject: Request to Remove Transcript Notation and Partial Tuition Refund

To whom it may concern,

I am writing to formally request the removal of late withdrawal notation (DR) from my academic transcript and a 25% tuition refund for RELS 163, taught by Professor S. Xavier. As mentioned below, during this Fall term, 1) my official accommodation with Queen’s University was not allowed by the instructor, and 2) my failed grade for the test was available on November 28th, one day after Queen’s official withdrawal deadline for avoiding a transcript notation.

Problem with accommodations:
Even though my accommodation is officially registered with Queen’s University, I was not granted additional time for the first submission. I was also not granted an extension to write the Oct 9th test despite having shared my mental health condition (recurrent panic attacks) and offered the instructor a physician’s statement.

Problem with the date of graded and returned work:

The instructor took 3 weeks to grade the test that was written on Oct 9th. For this assessment, worth 20%, grades were made available to students on Tuesday, Oct 28th, the day after the official deadline to not receive a transcript notation. Had grades or feedback been communicated by or before October 27th, I would have withdrawn by the deadline, avoided the transcript notation and received the 25% refund. I officially dropped the course immediately upon learning of my failing grade and course average.

To support this petition, I could send:
• Email correspondence documenting my accommodation requests and communications with the instructor (and TA).
• A doctor’s note supporting the impact of my anxiety and need for accommodations.

I would also like to emphasize that my accommodation for extended time has been officially approved by Queen’s University, and that my anxiety disorder is well documented with the institution.
