Submit an Academic Petition or Appeal

Petition Letter: On 9:30 a.m., December 2nd, I had a scheduled in-person deferred assessment (anatomy practical) for ANAT 380 by Dr. Leslie MacKenzie. Unfortunately, I experienced an unprecedented medical incident and had missed my assessment. I promptly contacted Dr. MacKenzie and informed him of this circumstance, inquiring about the possibility of a rearrangement, which was denied. While I understand that this is an entirely appropriate and justified action in line with the university's policies and regulations as well as the professor's rights, I believe that the unexpected nature of the medical event would be an appropriate reason to obtain an alternative arrangement.

I met with Nurse Practitioner Heather Geddes at Student Wellness Services, who has been monitoring my health over the past several years. Over the past several weeks, significant, on-going health challenges and medical incidents have severely interrupted and interfered with my academics and ability to meet commitments, such as this assessment. I am also in contact with QSAS and the CARE Team at Queen's University, who can attest to my current circumstances and provide more confirmation if necessary. Heather Geddes, Michelle Verbrugghe of the CARE Team, and Dr. Nikki Philbrook are all highly informed of my current health status and the impact this has on my ability to meet academic obligations outside of my immediate control. This is in line with the “Student At Risk” Policy. This can be verified by Michelle Verbrugghe as a member of the Assessment and Care Team (ACT).

Should my appeal be granted, the individuals listed previously, my family, and I have a long-term plan to help me achieve my academic goals. Because of the ongoing and severe nature of my circumstances, we have decided that taking voluntarily medical leave would be an appropriate action to take, with future courses during the winter assessment being online instead, as these would be an appropriate fit for my program and health needs. Michelle Verbrugghe has informed me that the university can accommodate for in-person, proctored exams and move these online, although I understand through my discussion with Dr. MacKenzie that the missed practical would have to be in-person, as this was the original exam format; this will not be an issue for me. I have been in regular communication with my medical team and support services, who are able to monitor my health remotely as well and through communication with external professionals.
