Petition Letter

Petition Letter: Dear Members of the Academic Petition Committee,

I am writing to formally submit a petition requesting approval to drop the course CHEM 112 and to have the midterm examination for the course PYSC 100 reevaluated.

Due to the illness, mononucleosis, I was unable to attend several labs and unable to attend lab make-up week to complete the requirements for the course. I had submitted my extenuating circumstances which had been approved. I had gone to the doctors to be diagnosed with mononucleosis on November 28th by Dr. Brian Trickey and was instructed to rest and recover for 1-3 weeks demeaning necessary. I was very ill and did take the full 3 weeks to recover which included the lab make-up week and exam on December 7th. With this I consulted Alaina Boyd and she had advised me to request a course drop with my extenuating circumstances to prevent penalties.

Due to the same illness, I was unable to attend my midterm exam for PYSC 100 on Dec 5th. I had emailed my professor and my QSAS advisor regarding an exam deferral but was met with no response from my professor. I was told by my QSAS advisor that all I had to do was email my professor and include her within the email. I was told by my QSAS advisor I could have my exam deferred due to the doctors note and extenuating circumstances, therefore be allowed to defer my exam.
