To the Academic Appeals Committee.
This statement provides a formal and detailed description of the medical reasons that prevented me from finishing the final exams of ECON 222, STAT 263 and FILM 111. It is submitted in support of my request for a second formal deferral or, in different instances, an approved alternative examination plan.
The reason why I was unable to complete the examinations on time, which were at the end of December, was that I had a diagnosed and clinically significant anxiety disorder. I consulted with Queen's University Accessibility Services immediately after this diagnosis and my faculty advisor to get the proper direction. I also applied and received an official first deferral. The university policy stipulated that all my examinations on all three courses were formally rearranged to the requisite deferral examination period of January 16 to January 20.
As part of my treatment plan, I was required to visit my specialist physician in China in January and have a follow-up appointment. This played a critical role in the successful management of my condition, supporting my return to studies. This was what made me go back to China. The medical documentation I have submitted, signed by my physician, confirms that I was prescribed mandatory medical rest and treatment from January 13 to January 27.
The entire scope of the rescheduled examination (January 16-20), fell within prescribed period medical incapacity. Due to my location and the explicit instructions from my physician, I was medically unable to attend the examinations. Attending would have been a disobedience to the medical recommendation under which I was, and would have negated my treatment. Therefore, my absence was an unavoidable consequence of following my prescribed treatment plan.
I hereby officially petition a second deferral of my exams in ECON 222, STAT 263 and FILM 111. I respectfully request that my examinations be rescheduled to the next available sitting, in accordance with Queen’s regulations. In this regard, I will provide any other documentation that the university might wish to have at its disposal as soon as possible.
I am eager to resolve this problem and finish all the unfinished exams in a manner that will meet my medical needs and Queen’s University’s academic standards. I would like to thank you for your time and consideration of my petition.
