Petition Letter: Dear Associate Dean (Academic),
I am writing to submit a petition regarding PHYS 115 (Fall 2025) and to request academic consideration due to documented medical circumstances beyond my control that directly affected my ability to complete required academic work during the examination period, including my deferred final examination. On January 15, 2026, I began experiencing severe migraine symptoms and was assessed at a walk-in medical clinic. At that time, my condition rendered me unable to attend my scheduled laboratory session or participate in academic activities, including lectures. Medical documentation confirms that I was unwell and advised to refrain from academic work, with an initial expectation that I would return to full capacity on or after January 17, 2026. Despite this expectation, my condition persisted and worsened over the following days. By January 16-17, 2026, my migraine symptoms had intensified significantly. I experienced severe headache pain, light sensitivity, inability to tolerate screen use and difficulty concentrating, which made it impossible for me to study or prepare for, let alone complete, my deferred final examination. On January 17, 2026, I was reassessed at a medical clinic due to the worsening of my symptoms and was diagnosed with a persistent migraine that had been ongoing for two days. As documented by the attending nurse practitioner, it was recommended that my deferred final examination be rescheduled. At that time, my treatment plan was adjusted, and I was prescribed medication to manage my condition, as initial recommendations of rest and over-the-counter medication had not been effective. These medical circumstances occurred during a critical academic period and directly affected my ability to complete the required assessment for PHYS 115. I sought medical care promptly, followed medical advice and have provided supporting documentation to substantiate the impact of my condition. I respectfully request academic consideration in the form of approval to reschedule my deferred final examination for PHYS 115 in accordance with Faculty regulations. I remain committed to completing all outstanding academic requirements once medically able to do so.
