Subject: Formal Complaint: Destruction of Original Academic Records and Lack of Registrar Response

I am writing to formally lodge a complaint against the Office of the University Registrar regarding the destruction of my original Venezuelan academic transcripts and their subsequent failure to provide a solution or a timely response to my inquiries.

Background
In approximately 2016, Los Andes University (Venezuela) sent my original, official high school and university records directly to Queen’s University for admission purposes. Recently, upon following up, I was informed by the Registrar’s Office that these original documents were destroyed due to an apparent decade-long retention policy.

Statement of Fact and Impact:

Irreplaceability: Due to the ongoing socio-political crisis in Venezuela, the institutions that issued these documents are either closed or non-functional. I have no remaining family in Venezuela to assist in retrieving new copies.
Lack of Notification: I was never contacted regarding the intent to destroy these original documents, nor was I given the option to have them returned to me at my expense.
Professional Hardship: I have recently secured a job offer that is vital to the survival of my family, including my two minor children with special needs. This job requires immediate verification of my educational history. I have spent the last six months on Ontario Works (OW), and this employment is our only path to financial stability.
Lack of Communication: I have sent several emails to the Registrar seeking a resolution, but I have received no response.
Requested Resolution
Because Queen’s University assumed custody of these original, irreplaceable documents and subsequently destroyed them without notice, I am requesting the following within five (5) business days:
A Formal Attestation Letter: An official letter from the University stating that I met all prerequisites for admission to Queen's University.
Verification of Credentials: A statement confirming that Queen’s University received and verified my original transcripts from Los Andes University, detailing the degree/attainments recognized at that time.
Emergency Handling: Due to the risk of losing my employment offer, I require an immediate point of contact to ensure my employer’s verification requirements are met. And to be contacted at my personal email as I am not longer a Queens student.

Closing
I am holding Queen’s University responsible for the loss of my original records. This documentation is not just a formality; it is the key to my family’s ability to sustain ourselves. I look forward to your immediate intervention to rectify this administrative failure.
