Petition Letter

Petition Letter: This letter will be structured in:
1. General information
2. The Turn of Events
3. Contacts for evidence

1. General Information:
Complicated Situation: missed assignment: Assignment Case Study Program ----25%

Missed reason

Mental health- Depressed, struggles with ADHD, (and other more confidential information), and was unable
to attend class for a whole week prior to the event. (Confirmed by Mental Health Councillor)

Turn of event:
1. Missing from class for a whole week, recovered to go to class
2. realized that during my depressed week I missed the in class case study (1 day after the due date)
- at this point I write an email to the professor, asking for an opportunity to make up for it
3. Next day I go in person to explain and discuss for a compromise and to explain the situation.
She replies with a no. (2nd day after due date)
4. I go to mental health to see if there is valid grounds, (sharing more personal details, in confidentiality),
for an academic consideration. (Same day as when I meet with the professor).
The guidance councillor gives me the appointment and after discussion, sends in a request to show
that I am in valid standing for an academic consideration (2nd day after due date)
5. I attempt to study more for the class trying to make up for it, attending the final exam
and any other work, unsure when and if the consideration will be fully confirmed and
sent to my Queen's Outlook.
6. The Email confirming and holding the academic consideration document never arrives in my Outlook.
7. After final exams, I received a failing grade. Later on while ranting about my situation I check
my Telus mental health site to discuss how difficult and difficult accessing Queen's' mental health is. I find
that I was granted an Academic Consideration to take the assessment, as I attempt to log in and scroll through the initial response form. (End of Fall Semester) (This is recorded-video, due to me ranting and upset about trying to access mental health support)
8. After Realizing this I bring it up to my guidance councillor, Karina Almeida to ask for support, as I did well
in my midterm scores. Losing 25% on an in class assignment and thus failing the course does not show academic honesty from the University's side, of providing a learning space to grow academics and to allow students to show their learning and in class participation.
- I want to note that I went to all the in person lectures, took notes, and sat at the front attentively, except
for that class (of which for the entire week I missed most of my classes due to my situation), showing that I cared about the class and academics, so when I requested for this I was in good faith and my professor would know that I wasn't just using an excuse.

People who you can contact to give you more information or back my list of events
- My Friend from PROF class - Dax Mazzuchin: 23BBG@queensu.ca
(he saw how shocked I was and how screwed I felt)

- My Mental Health confirmation for Academic Consideration
(Confirm that I received a valid consideration that was valid and I could have used)

- My Kinesiology Guidance Councillor, Karina Almeida: karina.b@queensu.ca
(That I am genuine and that these were indeed the events that happened and that I asked for her support)

- My video of me realizing I had the academic consideration but didn't use it. (I will share this in person if needed)

- Christina Prevett PT, PhD
(That I attended classes attentively and was there for most/all classes prior to that week and that incident)
(I also have emails about this event confirming that I sent her emails on those time frames, and she sent
me an email after discussing with me, when I went in person.)

I am requesting this as the course is mandatory for my program and I feel that retaking the course
for a missed assignment (that I received valid academic consideration for), then missed due to not
receiving an email (the main form of contact at Queen's), due to the difficulty and ambiguity on how to
access mental health and official documents led me to not have any fair opportunity to do the make up,
which is not true to academic and educational integrity of Queen's or any learning institute, and my effort
in the class.
