Submit an Academic Petition or Appeal

Petition Letter: Dear Associate Dean,

I am writing to respectfully petition for an extension of my deferred exams for CHEM 112 and PSYC 221 due to an exacerbation of a mental health condition for which I am registered with QSAS.

My condition significantly worsened and impaired my ability to function academically and personally. Until very recently, I genuinely believed I would be able to write my deferred exams as scheduled, and I continued attempting to prepare for them. However, my condition deteriorated to the point where I was no longer able to complete the exams or effectively manage the petition process within the expected timeframe.

I sincerely apologize for submitting this petition late. The delay was not due to neglect or lack of responsibility, but rather because I was not in a functional state over the past few days and was struggling to manage even basic tasks. As soon as I was able to do so, I prepared and submitted this request.

I am registered with QSAS and can provide additional information regarding my condition, its recent exacerbation, and the circumstances that contributed to it if further detail or documentation is required. I am also prepared to provide any supporting documentation requested by the Faculty.

I respectfully ask for consideration for an extension of my deferred exams so that I may complete these courses once I am medically able to do so.
