Petition Letter: Dear Members of the Petitions Committee,
I am writing to respectfully request consideration regarding my academic performance.
During this period, I have been affected by depression and complex post-traumatic stress disorder (CPTSD). These conditions have significantly impacted my executive functioning, concentration, time management, and ability to complete academic work consistently.
My treatment was delayed because it has been difficult to distinguish between ADHD-related symptoms and CPTSD-related symptoms. In addition, concerns related to my heart and thyroid condition required further medical consideration before medication could be started.
I am currently registered with QSAS. I have recently started medication, and with the support of counselling, my condition has begun to improve. I also have an appointment with my doctor on June 16, 2026, during which I will obtain more complete supporting documentation for QSAS for permanent accommodation.

Academic Plan
Academic Plan: My plan is to continue actively addressing the medical and mental health issues that affected my academic performance. I am currently registered with QSAS, have recently started medication, and am receiving counselling support. My condition has begun to improve, and I have a doctor’s appointment on June 16, 2026 to provide a fuller explanation of my condition and obtain more complete supporting documentation.
Going forward, I will continue following medical advice, attend counselling regularly, and work with QSAS to ensure that appropriate academic accommodations are in place. I will also meet with an academic advisor if needed to review my course load and academic options.
To support my academic success, I will create a more structured weekly study plan, break assignments into smaller deadlines, and communicate with instructors earlier if my symptoms interfere with my ability to complete work. If my petition is approved, I will use the additional opportunity to complete my outstanding academic requirements in a more stable and supported way.
