To Whom it May Concern:

My name is Hannah Faulkner and my student number is 10150269. I am a second year student in Concurrent Education at Queen's. I am writing an appeal for Aegrotat Standing.

Since July of 2015 I have suffered from a progressive medical condition, which has resulted in the loss of the muscle control of both of my arms. This has had a significant impact on my ability to care for myself and complete simple tasks such as note taking, personal grooming and meal preparation. During this time I have been in contact with Sara Horsford (Personal Counsellor, Faculty of Education) and Lee Fisher-Goodchild (Student Accessibility) in regard to my condition. I had been granted accommodations for my school work during the term, but I tried to persevere and did not ask for an extension on any essay prior to my hospitalization. I had also been granted accommodations in place for my exams in December. I attempted to continue with my studies for as long as possible but on November 25, 2015, after an appointment with my neurologist, I was admitted as an inpatient at the Civic Hospital in Ottawa. I am currently undergoing treatment there. I have been in the hospital since that time and I must continue to be a patient for the weeks to come.

I hope that my treatment is successful and that I will be able to return to Queen's in January of 2016. The doctors at the Civic Hospital hope that I will be able to continue treatment and rehabilitation in Kingston should my initial treatment be successful. I am able to receive notes from my classmates for week 12 and I am trying to stay in regular contact with my Professors and Tutorial leaders so they are aware of my medical situation at this time. I am also currently attempting to finish the last of my course essays in order to complete 60 of my course work. The idea that I am not able to attend Queens or do my exams is causing me added anxiety and stress. I need to be able to focus on my treatment and my rehabilitation phase in order to regain my health. At this time, I require your support in order to do so.
