RE: Academic Appeal.

Dear Dr. Bénard,

I am writing this letter to request permission to drop FILM 303, FILM 351, and FILM 312 which were held during the Fall Term of the 2015/2016 academic year.

As a result of extenuating mental health challenges, I have been unable to complete the required course assignments outlined in the course syllabi during the Fall Term. Since October 2015 I have been struggling with my mental health; it has greatly impaired my daily functioning. I was unable to attend a number of classes and consequently my academic performance has been very poor.

In November 2015 I sought help from Student Wellness Services to aid in my recovery and treatment for my mental health challenges. I attended weekly appointments with a personal counselor to develop coping skills to manage my health challenges. Furthermore I have taken the proactive steps to seek ongoing treatment outside of Student Wellness Services to ensure that I will be able to successfully manage my health challenges long-term. I am confident that with ongoing treatment I will be able to complete the Winter Term courses that I am enrolled in.

However, in the event that my health challenges continue to have adverse effects on my academic performance I will make certain that I drop any courses prior to February 26th 2016. As an upper-year student receiving an incomplete or failure on my official transcript will be detrimental to my academic career as I intend to pursue postgraduate studies. I greatly value my academic achievements and I have worked hard during my studies at Queen's University. As a part of my academic plan I intend to retake the aforementioned courses during the 2016/2017 academic year should my request be granted.
