Submit an Academic Petition or Appeal

Petition Letter: Dear Associate Dean,

My name is Sarah Hanna and I am going in my second year of Arts and Science, on Academic Probation at Queen¿¿s. I am applying to receive Aegrotat (AG) standing in DEVS 100 and to receive Credit (CR) standing in PSYC 100.
I am the sole support parent of my 2-year-old daughter, Ariella. My 2014-2015 year at Queen¿¿s was my first post-secondary experience, and my first time back to school since 2010. I was prepared for changes to my routine with Ariella, and to pursue new challenges academically. However, due to a series of extenuating circumstances occurring throughout the second semester, I was unable to perform to the best of my abilities. I was in an abusive relationship with Ariella¿¿s father, and since have struggled with anxiety and depression. In January of this year, I found out that I am once again pregnant, with my second daughter. The combination of my mood coupled with being very ill throughout my pregnancy, made it extremely difficult for me get to campus. Most, if not all, of my classes last year required me to attend tutorials and lectures in order to succeed in the finals.
I am applying to receive Aegrotat (AG) standing in DEVS 100 as I was unable to attend the exam due to being extremely ill. There should be my Doctor¿¿s note on file. If not, I can have another copy made. I have opted for the Aegrostat option, instead of writing the exam because my daughter finished daycare very shortly after my exam period, and I have no other means of supervision or daycare for her.
I am applying to receive Credit (CR) standing in PSYC 100 because I would really like to continue into the psychology and sociological fields, to pursue my interest in social work. Once again, being unable to attend my classes severely affected my success. I did not expect to fail in Sociology.
I did get in touch with the BanRigh Centre on campus at the end of the second semester last year. Unfortunately, I had not known about their services prior to that time. This year, the Centre has been a very large part of my support system.
Thank you for your time and consideration. I realize this letter is extremely late. I apologize, I was under the impression I had submitted this to you in June. Constant and extreme illness had kept me from checking in on Solus sooner. Once I realized it had not been submitted, I was unable to submit the appeal due to the financial end of the appeal. At the beginning of this academic year, I had my baby six weeks early and I have been monitoring her for Failure to Thrive for the last several months. Although she is now recovering from FTT, she was hospitalized at KGH on the 7th of January with Bronchiolitis. Amelia is now home and recovering well.
