Petition Letter

Petition Letter: To Whom It May Concern:
My name is Melissa Roberts and I am currently in my third year at Queen¿¿s University working towards my Bachelor¿¿s of Honours in Political Studies. I am from Vancouver, BC and I am writing to request that I be able to complete my fourth and final year at the University of British Columbia due to personal health matters.
As of May 2016, I will need to complete an additional 30 credits to fulfill the required 114 credits needed to receive my BAH. Since I was 16 years old I have suffered from severe migraine headaches which have lead me to miss two months of my grade 11 year as I went from one doctor to the next finally seeing a neurologist at UBC Hospital in 2012 where I was diagnosed with Chronic Migraine, meaning that more than 50 of the days in a month I have a migraine headache. I have been treated with various prescription medications for this and I have also tried receiving help from registered massage therapists, cranial massage therapy and acupuncture. Unfortunately, none of these have worked very well for me. In August 2014, I began receiving Botox injections from my neurologist to help with my migraines. While this has been the best pain reliever so far, I still suffer a great amount from my migraines. While I have sought help from a neurologist here in Kingston, he advised he does not administer Botox and therefore when I cannot be home in time for my every 3-month appointment, the pain becomes very difficult to deal with. It is for this reason that I wish to spend my final year close to my support group, including my Neurologist and my family doctor, so that I can receive their continued support in helping me find a way to combat my migraines. I can provide a written letter from my neurologist back home, Dr. Sian Spacey if need be.
Thank you for taking the time to review my request and I would really appreciate it if I could be back home in Vancouver for my final year of studies, in order to work closer with my neurologist and family doctor.
Sincerely,
Melissa Jayne Roberts Student 10072518

Academic Plan

Academic Plan: To Whom it May Concern,

After speaking with my Undergraduate Chair, Professor Jonathan Rose he has approved my request to complete my fourth and final year at UBC to fulfill the credits required for my degree program. Below is a copy of his email written to me:

Dear Melissa:
After speaking to you today about your health issues, I am satisfied that it¿¿s necessary for you to take your fourth year POLS courses in BC, either at UBC or SFU.
We normally do not allow our majors to take their fourth year elsewhere but given your need to be close to health care providers in BC, I think that this is warranted.

Good luck in your studies and please do keep me posted about which courses you will be taking.

Sincerely,

Jonathan Rose
