Petition Letter: Dear Dr. Johanne Benard,
I am requesting to receive aggregate standing in BIOL 322- Environmental Physiology of Animals taught by Dr. Yuxiang Wang. Last March (2015), I abruptly halted my studies and had to return home due to severe mental illness and extenuating circumstances. Prior to leaving, I consulted a counsellor at Queens, and the Psychology Undergraduate Chair, Alison Horwood, who received my documentation and contacted appropriate professors. I was under the care and direction of my family doctor, a psychologist, and the out-patient program for a psychiatrist at Southlake Hospital. Additionally, I developed cyclic-vomitting syndrome in May, which caused frequent visits to the hospital every 3 weeks when an episode would present. I had arranged and completed 4/5 of my Winter 2015 exams prior to mid-September; however, due to scheduling conflicts, I was unable to write the BIOL 322 exam. Furthermore, I was unaware that after a semester, incomplete grades change from IN to F. I completed 60 of the course work for BIOL 322 in March, and am looking to receive aggregate-grade standing, which has been approved by Dr. Wang.
