Dear Associate Dean
I am submitting an appeal for late withdraw from two courses, CLST 103, CISC 121 for the winter term of 2016. The extenuating circumstances leading to this are psychological depression/anxiety. These symptoms caused me to fall behind in my classes, and return home from school on March 3 2016. I started to feel very sad and not committed to my courses in the Fall 2015 term but continued my studies. I didn¿¿t do well in my Fall courses, by my standards. Over the Christmas holidays I talked with my parents and decided to start the Winter term to see if I could get back on track. You should know that before this school year I was always a very committed and contented student. I took pride in my work and always met challenges and deadlines.
My depression continued in January and February and this led me to fall behind in my courses again. I returned home where I am feeling more comfortable and upbeat. I visited my family doctor twice, and she diagnosed me with depression/anxiety. The doctor prescribed counselling, and I plan to see a counsellor as soon as I can get an appointment. My future plans for academic success are to return to school in fall 2016, with hopefully an upbeat attitude and clear mind. I will also be changing programs from Mathematics to another major, possibly Biology. I will discuss this with an Academic Advisor.
I have attached a letter from my family Doctor who has diagnosed the depression. If needed I can provide a letter from the counsellor after I have met with him.
