I am writing to you to submit an official appeal of the Faculty's decision asking me to withdraw from Queens. I am a proud Queens student and I have no intention of giving up my dream of graduating from this University. I have been dealing with some ongoing personal issues that have had an impact on my studies. Although this past year had its challenges, I am doing my best to get things right and move forward. Sitting out or changing schools is not what I feel is in my bets interests.
In the summer of 2013 I experiences some personal problems and decided to seek out medical help. I began a program with Dr. Szkiba-Day, a psychologist in Oakville, Ontario. I was diagnosed with depression, anxiety and ADHD. I was relieved to find out what was happening but I had no idea how to cope with these challenges. I returned to Queens last September but I struggled with my academic work. I began a program with Dr Szkiba-Day to learn strategies to help cope with depression and anxiety to be able to function at school. I went though some difficult times dealing with the anxiety and long bouts of insomnia. I decided to keep things private for the first term because I did not want to draw attention to myself or have my team mates feel self-conscious about me.
The strategies that Dr Szkiba-Day taught me have had an impact on my daily life and did help me with my school work. I subsequently reached out and alerted Gail Smith from Counselling Services at Queens that I have been having problems and she has been supportive of me and my situation. After a great deal thought I approached the coaching staff at Queens and alerted them to my situation. I am happy that I did. They are committed to helping me through this.
My time with Dr Szkiba-Days has taught me that there is no shame in having difficulties of this sort. What I have is treatable and should not impeded me from achieving my obtaining my goals. I am anxious to get to work on re-building my academic profile with a clear focus. I am fully committed to using all available resources to and obtain my degree from Queens.
I respectfully ask for your support and to be re-instated and be allowed to continue here at Queens.

Academic Plan: Academic Plan for 2014-15
Goal: To re-establish my academic profile with a 2.7 GPA and obtain my Bachelor of Arts degree .
Strategies for Success
1) Consistent use of prescribed medication to manage and minimize the impact of ADHD and improve my ability to focus.
2) Continue to work with Dr Szkiba-Days and work on techniques that will help me cope with my issues
3) Have regular meetings with Gail Smith in the Queens Counselling Office to deal with issues if the occur
4) Work with Athlete Services Coordinator Andy Goodspeed with regard to tutoring opportunities and academic support through the athletics department
5) Have regular meetings with Head Football Coach Pat Sheahan, a Masters of Ed grad who will assist with academic support
6) Improve writing skills - Queens Writing Center
Challenges/Opportunities
1) Alert my support team at the first sign of difficulty.
2) Be sensitive to any adverse effects of medication.
3) Be organized and get lots of rest.
4) Stay in top physical condition and maintain a healthy lifestyle.
5) Work with advisors at F-200 to be well organized for academic success
6) Pursue more history courses
7) Obtain a 2.7 GPA
Final Comments
I really enjoy history classes and I hope to take more of them moving forward. I plan on taking good care of my health and allow myself the best chance I can to succeed. I also look forward to working hard at becoming the best student-athlete I can be at Queens. The football team is my family at Queens. I will not let them down.
