Petition Letter: Dear Faculty of Arts and Sciences, Academic Appeals

Re: Blake Mitchell Wilson, student number 10150178


I am writing this letter to formally request an Aegrotat appeal for three of my second year exams due to a concussion sustained on April 2nd 2016, and my ongoing inability to study and concentrate due to symptoms including headaches, fatigue, light and sound sensitivity. Initially, I was granted accommodations on April 5th, by the Queen¿¿s Accessibility Services, whereby, I was in regular contact with Leslie Paterson. We arranged to defer my exams to mid-to late May and I proceeded to contact my professors accordingly.
Unfortunately, my condition has not improved as we had expected, and I do not feel I will be able to study and effectively write all five of my exams. I believe that if I am able to reduce my exam load and reschedule my other previously deferred remaining exams to a later date, even possibly September, I will experience less stimulation, stress and cognitive demands. I will be able to focus more effectively on the two remaining exams.
The courses I am requesting an Aegrotat for are: Devs 240 (completed 70 of the course) taught by Professor Paritosh Kumar; Econ 250 (completed 60 of the course) taught by Professor Gregor Smith; and, COMM 200 (completed 70 of the course) taught by Professor Gary Bissonette.
Professor Kumar (Devs 240) is leaving the country May 14th for the summer and there will be no possibility of rescheduling his exam in the unlikely event that my Aegrotat is denied. Professor Kumar has supported my decision for this appeal via his e-mail dated May 4, 2016.

Please see the attached letter from my family doctor who has been monitoring my condition since my return home to Toronto.

I am hopeful that the Faculty of Arts and Sciences Appeals Office will support me with this request given my condition and supporting documentation provided from both my professors and family doctor.
