Petition Letter

Petition Letter: To Whom It May Concern,

I am an Arts and Science student and would like to appeal for credit standing in CHEM 112 for the following reasons.
On the day of Tuesday, April 11th, 2016, I wrote a CHEM 112 exam. Both physical and psychological limitations negatively impacted my ability to focus on academic tasks, causing me to receive a grade of C (an unusually low mark for me as evidence by my academic standing in my other classes). Please find attached the doctor's note from my hospitalization of April 11th and 12th due to a confirmed case of influenza B. Note that the doctor's note is dated April 12th as it was well past midnight after I had been seen, x-rayed and given IV's when the note was written.

I experienced the onset of influenza symptoms 3 days prior to the CHEM 112 exam, but attempted to push through the fever and illness to study and write the exam - in hopes to complete my year on time. During this time, I experienced a lack of sleep, pain, and confusion and as a result went to the hospital to seek medical attention.

Furthermore, I was also dealing with the added stress of my grandmother - that I used to live with - having a heart attack and being hospitalized and suffering a head injury as a result of falling on the bathroom tile floor.

Due to my illness and psychological stress, I was far from my best in being able to think and remember clearly which manifested itself as a low grade on my exam, causing my course mark to drop dramatically. As a result, I also had to be excused from my Calculus and Biology exam which I subsequently wrote at a later date (April 22nd, and May 9th respectively).

Please accept this information as my input for the appeal for credit standing in CHEM 112.
