Petition Letter: May 12, 2016.

Dear Sir or Madam:

I am providing this letter to make the following appeals:
1. Drop course MATH 281 past the academic deadline.
2. Drop course MATH 385 past the academic deadline.
3. Credit Standing in MATH 212.

I would like to appeal because of an ongoing health issue that significantly affected my ability to successfully prepare for exams.

In the latter part of March, I developed a chest infection that kept me from sleeping at night. I sought medical treatment from my physician in Oakville on March 26 and was prescribed medication that did not help. I also discontinued ongoing medication I take daily as the cardiac effects could be worsened by the medication prescribed for the chest infection. This medication supports my ongoing academic success as I have been diagnosed with ADHD.

I continued to have symptoms and I again sought treatment in Kingston on April 8, the day of my MATH 385 exam. The physician in Kingston prescribed a course of treatment for pneumonia.

The combination of a severe chest infection and not being able to take regular medication had an adverse affect upon my overall wellness for exam preparation. I wrote exams and attempted to do my best, but was not successful and ended up failing the exams for MATH 281 and MATH 385, and the courses overall. I was successful enough to receive a passing grade in MATH 212, but that was not the grade I would have received had I otherwise been well.

I have attached supporting documentation related to my illness, including my correspondence with Professor Alexander.
