Dear Sir/Madam:

Please find attached an academic appeal ¿¿ late drop MATH 121 and CHEM 112 and credit standing for BIOL 103 for Meghan Whitsitt ¿¿student number 101182685. A written letter from Dr. Carol Thomson, Meghan¿¿s doctor, will follow outlining circumstances and supporting this appeal.

Meghan Whitsitt had significant health issues that were debilitating to her academic performance in the second semester of her first year at Queens. Meghan met with her doctor in December 2015 and the doctor provided medication for her depression and anxiety and insomnia to cope with her health issues. Meghan Whitsitt met with Spring Forsberg, Student Services, in February 2016 to advise Ms. Forsberg of Meghan's difficult time adjusting to life at Queens and her struggle with anxiety, depression and insomnia and Ms.Forsberg reached out to Meghan's professors regarding her condition. Meghan also met with Queen's wellness center to seek official accommodations and ongoing support with Counseling services. Meghan received support and counseling from Morneau Shepell regarding her health condition. Meghan met with her doctor again in February for further treatment. Meghan was not able to perform academically in her courses in semester 2.

Meghan is receiving medical support and care from her doctor from December 2015 to present.
