Petition Letter: To the Associate Dean at Queens,
I am formally requesting an extension until August 30, 2016 (see all attached materials and my assignment) for my incomplete (IN) for Winter 2016 course HIST252.
Although I made arrangements with my professor to submit my final materials for August 15, 2016 due to some difficulties I was having related to a learning disability, extenuating circumstances involving a significant mental health challenge created an obstacle to our arrangement -- I was unable to complete the terms of our arrangement on time. I enrolled in HIST252 after taking a full semester off from Queens University. I had taken this semester off to deal with the mental health difficulties I was experiencing. While I thought I was ready to take this one class in Winter to prove to myself that I could do school again--my previous mental health issues were not fully resolved by the time I enrolled.
An increasing amount of anxiety built up and inhibited my ability to fulfill the class assignments or hand in the ones I had completed. Some of these assignments I actually completed but was too nervous to hand in. Finally I got permission to hand in my final individual paper, but again my mental health difficulties got in the way, and I never handed it in, despite working on it.
Earlier this summer when I confessed some of this to my parents and psychologist Dr. Morisano (see attached letter of support), I was encouraged and advised by them to request an appeal for an extension and extenuating circumstances related to exceptional mental health difficulties this summer. My professor was kind enough to grant me an extension. Despite this, I struggled and I denied myself the ability to redeem my failure in the course by simply submitting this and seeking help through the Queen¿¿s disability services. Instead, I created a mental bubble that prevented me from helping myself--I believed that I was not deserving of getting credit for the course.
I have finally confessed all to my treatment providers, accessibility services, and my parents, and have come to terms with what I have done. I now hope to get a chance to submit my final assignment (attached) to allow my professor, if she is willing, to amend my final incomplete grade.
Having my final assignment graded, potentially allowing me to pass the course, will allow me to one day return to Queen¿¿s. I know I had been given a chance to complete this before but now I want to truly redeem myself to my professor and Queens and show that I can amend my errors and complete the work I am assigned, I understand I am not ready to attend full time studies and will be taking this upcoming year to recover more fully, but I know that I want to come back and better myself and prove that I am capable of getting my undergraduate degree.

I am formally requesting a late term drop for HIST260, due to continuing psychological issues that were present before the start of the class and impeded my ability to successfully complete my coursework (please see attached letter of support from my psychologist Dr Dominique Morisano). Before the class started, I had already been dealing with another class failure/incomplete (my Winter 2016 coursework was impacted by similar circumstances).
Against the advice of my colleagues, family, and treatment providers this spring, I felt that I could resolve my previous academic failings and redeem myself by passing one course which would allow me to return to Queens as a full time student. The class beginning in May began relatively well as I was able to hand in the first major project, but I ran into some minor difficulties as the course continued and projects accumulated. These problems might have been easily resolved under normal circumstances, by asking for help in a timely manner from the professor or the TAs. However, my previous academic failings and a feeling of continuous pressure to complete the projects inhibited me from both seeking help and acknowledging the path my course grades were taking. The idea of dropping the course earlier didn't occur to me because I believed that could pass the class, and that I could continue to attend my studies at Queens, which was my underlying hope. I have been seeking professional mental health assistance over this past year, and I had trouble coming to terms with my current state of health. I wanted things to be different and ignored the reality of the situation at hand.
Again the stubborn hope of being able to return to Queens next year as a full-time student where I can complete my degree is the backdrop for this appeal of a late-drop request. This would clear this failure mark for a course I was not prepared to enroll in and which I enrolled in against the advice of others from my transcript. I need this clearance in order to successfully continue at Queens. I need be able to focus on returning to university without the mark of continued failure on my mind or record.
