Petition Letter

Petition Letter: Dear Associate Dean,

I am requesting to extend an incomplete to finish a course due to ongoing illness. The course I would like to finish is GNDS 215. The course is a required course so that is why I need to finish the course. The ongoing illness is chronic fatigue and a Vitamin D deficentcy that is causing chronic fatigue. It is causing me to feel tired a lot and is affecting my stamina and so it is causing me to feel tired a lot and not able to perform academically at a fast pace and so I am slower to complete assignments due to my problem I just mentioned.
The chronic fatigue has caused me to sleep a lot. I feel tired a lot. I have to sleep a lot to feel better. I also have dyslexia a type of learning disability and anxiety. The learning disability and the anxiety also causes me to take longer to complete assignments so I need accommodation for the learning disability and anxiety. The chronic fatigue is ongoing and I have been having problems with this since 2006 for 10 years now. To be successful and to finish the coursework to finish the GNDS 215 course I will need accommodations to finish. The accommodations I will need to finish are extra time to finish until April 2017 next year. I will also need time in the Stauffer Library, Help from the writing center, and possibly a tutor or someone who can help with learning disabilities. I have a doctors note explaining the chronic fatigue the doctors note can be accepted at face value and has the information needed to appeal to finish the course. I am continuing to receive help from my doctor with the chronic fatigue and am taking a vitamin regimen. The problem is ongoing and it will need accommodation.
