Dear Dean,
Request:
I request a late academic withdrawal from Psyc 271 Brain and Behaviour, Spring Summer Session 2016.
Explanation of Extenuating Circumstances:
I found the course both stimulating and interesting, completing all of the required readings and assignments on time. Unfortunately, a chronic health condition was exacerbated during the late spring and summer resulting in frequent medical appointments (weekly), changes in medications and an inability to focus in preparation for the exam. With the support of my physician, I requested and was granted a deferral from the original exam date in July. I am continuing with bi-weekly medical appointments and am about to undergo an intensive daily course of treatment beginning September 19 for 8 weeks. I am unable to prepare for the final exam at this time. Until the treatment concludes mid-November, the timeline for preparing for and writing the final exam is unclear.
Future Plan:
As a part-time student, I have completed the requisite core courses for a Bachelor of Arts (BA) with a concentration in psychology. It is my intention to complete the final credits to round out my BA by completing one to two courses each calendar year while continuing to work full time in the software industry. I will be better able to determine when I can resume my studies after I complete my treatment in November 2016.
I thank you, your staff and Professor Monica Valsangkar-Smyth for your compassion and counsel during this difficult time for me and my family.
My Dr. is signing the form and I can submit it tomorrow or send it directly to the Faculty Office as required.
