Petition Letter: Dear Associate Dean,

I am writing to appeal for an AG in SOCY 211 and an extended IN grade for SOCY 227.

I was unable to write the final exam for SOCY 211 due to a physical condition that deliberated me from holding a writing utensil. I was admitted to Hotel Dieu's emergency services the night before my exam and I was affected for several weeks after that, the pain repeating intermittently. I have been to multiple clinics and hospitals, but they all have sent me home without permanent relief due to the fact that they could not figure out what the cause of the condition was. Although they scheduled me for an MRI, they have not yet contacted me. I have attached records from that day that proves that I was unable to use my left hand, which is my dominant hand.
Although I was granted a make-up exam, I was unable to attend that exam as well due to the same condition and my anxiety disorder.

I have been diagnosed with depression and generalized anxiety disorder. Having an anxiety disorder prevents me from doing everyday tasks, much less a high-stress assignment such as an exam. My body and mind start to shut down in panic, causing trembling hands, difficulty breathing and concentrating. Although this condition still affects me today,I am currently receiving treatment for it and finding ways to cope. I am taking medication, seeing a therapist on a regular basis and have applied for accommodations through Student Accessibility Services. I am learning to live and academically and to help myself even through difficult circumstances. I will attach a note from the physician I see for this condition after, for I do not see her until this upcoming Monday (October 3rd).

I have already received permission from Dr. Hand for more time to complete the take-home exam for SOCY 227. I am planning to have the final assignment finished by the end of October.

I am requesting an AG grade for SOCY 211 for these reasons, and an extended IN for SOCY 227, and I hope you will consider my circumstances to help me succeed academically. Thank you for considering these appeals.
