To the Associate Dean (Studies),

I am appealing to the Faculty of Arts and Science to register into the course, MUSC129 (Fall/Winter, 2016-17), that is passed the registration deadline. I am currently registered in first year in the Bachelor of Music program with a specialization on the piano.

Due to extenuating circumstances with managing my mental health and illness, I had to drop out of the advanced course for keyboard harmony, MUSC229. On September 28, 2016, a situation occurred between my professor and I in the course, MUSC229, which caused my anxiety to increase drastically. It has had a negative impact on my academic performance, as I have been struggling to gain motivation with my studies and attendance. Additionally, I have been struggling to gain self-confidence with performance and social interaction. Lately, it has been very difficult to manage my anxiety levels due to all the drastic environmental and social changes that I have had to adjust to with moving to a new city and new school. I have been in contact with Dr. Stephanie Lind and Dr. Cynthia Tormann to discuss the situation and seek advice.

On October 4, 2016, I sought help from my family doctor, Dr. Jennifer Boeckner. I have submitted documentation for a temporary IEP for the Fall semester, so that I can catch up with my studies.

Academic Plan

Academic Plan: A plan has been initiated to catch up in MUSC129 and to avoid this occurrence in the future. I dropped the advanced ear training course, MUSC227, to lighten my study load for this year. I hope to register in MUSC227 the following year to complete my ear training. MUSC129 is also a course that is designed for individual study over the whole year; moreover, I have an advanced ability in piano. In addition, these are the courses that I am taking for Fall semester: MUSC103, MUSC292, MUSC261, and MUSC120. Due to a lighter course load and the easier design of the course, MUSC129, it will be easy for me to catch up. I already have an advanced background with the material, and I have been practicing for this course on my own while waiting for approval. I hope to avoid this situation in the future through communication about my disorder with my professors.

Additionally, I have initiated a plan to help with my mental health. I have been taking a new medication that will help manage my anxiety levels. Near the end of Fall semester, I plan on meeting with my family doctor to see if there is an improvement with my mental health, so that I can make sure that I am capable of managing my anxiety levels for Winter semester. Also,
I am frequently seeing a chiropractor (Dr. Ali Houde-Shulman at Queen Street Chiropractic) to aid with nutrition, exercise, and pain management. I will avoid this in the future by managing my anxiety levels through various self and medical treatments.
