With utter most respect, I would like to appeal for the permission to drop from the following course for Fall 2016:

1. Chemistry 281- General Organic Chemistry

I request this due to my ongoing medical condition that is affecting my academic performance. My medical condition, which is beyond my control, is consuming a great amount of my time and energy.

I am affected in the lower portion of my body and inflicted with Inflammatory Bowl Disorder. Due to my illness, I suffered from severe abdominal pain, cramping and occasional weight loss which prevented me from regular attendance and participation in class. These symptoms can be confirmed by my Gastroenterologist specialist (Letter attached).

Since my condition is ongoing, and not under my control I was prevented from taking any action at the time of the drop date deadline.

I am prescribed on a medication to reduce the effects of the symptoms, but still, my medical condition interfered with my ability to commit my complete time, energy, and effort to this course (Chem 281). Hence, I request the permission of the course listed above to be dropped from my academic transcript.
