Submit an Academic Petition or Appeal

New Information: Hello, I stated in the section regarding the expected supporting documentation Ms. Tonya from the Student Wellness Services. However, this is untrue, Ms. Tonya did not submit any supporting documentation to my knowledge; however, I did see her. The person who sent the documentation was Ms. Carole-Anne. I forgot because I saw Ms.Tonya for four weeks in the summer of 2016 and Ms. Carole-Anne for only three weeks before the summer of 2016. During the Summer of 2016, Ms. Carole-Anne was not available, so I began seeing Ms. Tonya because she had experience with ADHD patients. Sorry for the confusion, but it should be in my documentation. Ms. Carole-Anne was the one who recommended my two appeals, not Ms.Tonya.
