Petition Letter: Dear Dr. Johanne Bénard,

I am writing to appeal for Aegrotat standing in ENGL 321, 356, 360, and 486 due to a debilitating substance abuse disorder, depression, anxiety. These health problems have caused panic, nausea, and fatigue, all of which have made it impossible for me to study for my final exams.

In order to address these issues, I have practised Cognitive Behavioural Therapy with Dr. Brad Mac Neil, a clinical psychologist in Kingston. I have also seen Dr. Nicholas Cristoveanu of the Frontenac Medical Associates, who prescribed me Cymbalta, an antidepressant medication I have been taking since October 2016. Also, I was referred to Addiction and Mental Health Services, where I talked to a drugs counsellor.

Despite these attempts at getting better, my health has progressively worsened over the course of the school year. This is reflected in my poor attendance, which has resulted in a general loss of participation and quiz marks. I continued to persevere because I thought I would be able to pass by the end of the school year, but unfortunately my health has deteriorated too much.
