Petition Letter

Petition Letter: I was in a car accident when returning home to visit my family, and have been diagnosed with a concussion by both Queen's Health Services and KGH Emergency Room. I was seen by doctors from Kingston General Hospital, and received a CT scan and a EKG. KGH advised me to refrain from all course work until I am recovered. Queens' Health Services has assessed that my concussion is severe and I am completely incapacitated as regards functioning at an academic level from March 25th, 2107 to June 30th, 2017. I have been suffering from noise and light sensitivity which make it impossible for me to use screens, such as my laptop, as well as be in public spaces. I attended classes after being diagnosed and found it extremely painful to sit in lecture. I am having difficultly with short term memory, concentration, information recall, and the processing of information. Doctor Anna Klein from Queen's Health Services has issued a verification of illness form to me, based on a functional impairments and limitations assessment.

I am asking for an AG standing as I will not be able to write my exam on April 20th. I have documented learning disability for which I have accommodations from the university to write exams in Gordon Hall using a computer and have extended time.I will not be able to write the exam as it will cause me excruciating pain due to light sensitivity and severely hurt my healing process. I wish to AG the course as I have completed 60 of the course work and allow me to focus on completing courses for which I have assignments outstanding.

Additionally, I have met with advisor from Queens Student Accessibility Services, Bonney Hunt bghh@queensu.ca, who is aware of my condition. She has been helping me process the information I need to create my claims, and breaking it down into manageable chunks with my condition.

New Information: LLCU 295

I am writing to apply for agerotat status for my LLCU 295 course with Professor Jennifer Hardwick. I have spoken to her in regards to this via e-mail.

I am not doing overly well right now with my councssion. Unfortunately when my parents were driving me back home to Toronto our car was t-boned and totalled. With my concussion still severe I am getting concussion testing by an ER nurse twice a day. I am in no position to be doing coursework right now and I apologize for that. It has been an incredibly difficult decision for me but I have decided to apply for an agerotat standing on a further class to removing stress from my life.
