Dear Dr. Benard,

I am appealing to have three failed courses from the Winter 2017 Semester removed from my transcript.

Last Fall I was hit very hard with depression and anxiety as diagnosed by Dr. Farnworth on campus. This greatly impacted every aspect of my life but most importantly for school - my concentration, memory and ability to work.

The courses I would like to have removed from my record are His 215; His 242 and His 341(A and B).

These failures were created by the same problems I experienced in the Fall as described in my earlier appeal letter of February 27/17. At that time I also dropped one course from my Winter semester in hopes that with a lighter load I would be able to function at my usual level but unfortunately I was not.

The steps I have taken to deal with my situation are as follows:

I am taking a prescribed ant-depression/anxiety drug as per Dr. Farnworth
I have met with my family GP while home for the holidays and he is referring me to a therapist near my home for regular weekly visits this summer.
I have started a regular exercise and vitamin routine.
I will assess my mental health this summer and discuss with academic counseling the best plan for me to move forward for the Fall ¿¿17 semester.

My plan is to work hard on getting back to myself, mentally and add a 5th year to my degree plan as I am very hopeful to attend graduate school once finished my degree at Queens. I appreciate your understanding on this matter as it has taken me totally by surprise and I am doing my best to deal with this difficult and often overwhelming situation.
