Dear Dr Benard,

I am appealing to receive Credit Standing for the following 3 courses:
MATH 210 Winter 2017
MATH 281 Winter 2017
STAT 269 Winter 2017


I suffered a head injury while at Queen¿¿s in mid-March 2017. My debilitating symptoms of double vision, headaches, fatigue, and difficulty concentrating made completing final assignments and organizing, studying for, and writing final exams extremely challenging or impossible. I also suffered from anxiety, irritability, depression and suicidal thoughts, which took away close to all of motivation.

I reached out to various medical professionals in March, April and May, including:
-a visit to the Emergency Deparment at KGH
-a family physician at the Queen¿¿s Health Services
-an ophthalmologist at Hotel Dieu
-my family doctor in Toronto
-appointment with a psychiatrist in June

I am expected to make a full recovery, although the exact timeline is uncertain. I am hoping to be fully functioning by the end of this summer, as I plan to continue in my 3rd year of studies at Queen¿¿s in Fall 2017.
